Malpractice Reporting Form

Pursuant to Georgia Law, O.C.G.A. § 33-3-27, all licensees must report to the Board within 10 days of payment, judgment or agreement, or parties in the claims.

 This is a report of malpractice claims.

The form and additional information should be submitted via NextRequest.
