Orthotist and Prosthetist

The following documents should be completed when applying for initial licensure as an Orthotist, Prosthetist or dual-license in the State of Georgia:

• Affidavit of Applicant  (PDF, 64.79 KB) - Upload to license portal in ThoughtSpan
• Citizenship Affidavit  (PDF, 324.58 KB) - Upload to license portal in ThoughtSpan
• References - use your application portal to enter references with current emails. An email link will be sent to the reference where they electronically submit the reference. 
• State Verification  (PDF, 123.09 KB) - Can be sent to the Orthotist & Prosthetist department at https://gcmb.nextrequest.com/ 
• Official Transcripts - Can be sent to the Orthotist & Prosthetist department at https://gcmb.nextrequest.com/ 
• Certification of ABC Examination  (PDF, 93.28 KB) - Can be sent to the Orthotist & Prosthetist department at https://gcmb.nextrequest.com/ 
• Work History  (PDF, 170 KB) - Can be filled out in application process.
• CV/Resume - Upload to license portal in ThoughtSpan
