Affidavit of Citizenship Status
During the 2011 legislative session, the General Assembly passed HB 87, “The Illegal Immigration Reform and Enforcement Act of 2011." The amendments in HB 87 altered the requirements for verifying the lawful presence of all applicants for public benefits, which specifically included professional licenses. These amendments became effective January 1, 2012.
In an effort to ensure a smoother renewal process for those yet to renew, the Board mailed out an affidavit to all licensee’s expiring between August 2012 and December 2013. Unfortunately inaccuracies in the letter have caused some confusion and higher than expected fax & call volumes, please be advised that:
- If your renewal is after January 2013; you don’t have to do it now, but we suggest no less than three months prior to your renewal to avoid being delayed.
- The fax lines for submitting the documents are: 404-651-7864 and 404-656-9723
- If you are a US citizen or non-citizen not residing in the US, you need to attach a copy of your driver’s license, military ID card, US passport, or any other photo identification issued by a US state, territory, or the US government.
- If you are not a US citizen and are residing in the US, you need to attach a copy of an INS issued document, such as your I-94, green card, visa, or other document with your Alien Number, date of birth, etc.
