Physician License Primary Source Documentation

Effective November 1, 2004 the Georgia Composite Medical Board will accept certain forms of Primary Source certification and verification for individuals applying for a Physician license. These primary source documents may be included in the application packet, only if the documents meet certain Board guidelines. These guidelines require that the applicant submit the documentation directly to the Board in sealed, unopened and unaltered envelopes.
