The following data elements found on the Board’s License Search site should be considered as primary source verification:
1. License number
2. License status
3. Issue date
4. Expiration date
In looking up a licensee by their license number, you will need to add leading zero's i.e. License #456 will be entered 000456 or License #45878 will be entered 045878.
